Insurance Cards/Co-Pays Please have co-pays with you on the date of service. Co-pays not paid within 24 hours of date of service will result in an additional $5.00 fee. Any delays with billing information may result in a balance immediately forwarded to the patient, as it delays insurance payments. Please clarify with your insurance company what your policy covers regarding physicals/yearly preventive screenings. Please tell staff before tests or physicals are performed.
