Associated Medical GroupMedication Requests and Refill Policy Please bring all over the counter (OTC) and prescription medications with you to your appointment. It is very important for our physicians to know what medications you actually take and what medications are prescribed by your physician. We will keep record of medications dispensed from another physician, but they must be refilled through their office. We request each patient bring in the actual medication bottles (not just a list of medications), as the bottles contain important information needed by our physicians. This also helps the AMG staff by minimizing phone calls for missed refills. Medications not reviewed at the time of your appointment are subject to a fee of $5.00 per medication requested via phone call after your appointment. Please allow 24 hours to have phone requests processed. All pain medications are monitored, and only refilled at your scheduled appointment with a signed Pain Contract.
